Start with a blank, spacious worksheet.
The printable version uses US Letter paper, selectable text, generous writing space, and the same prompts as the private builder.
Why this exists
Carry the words that may be difficult to find during an attack.
The card is designed for a moment when reading, speaking, or responding may be harder. Short instructions, one contact, and the location of medical information give another person practical context without trying to explain migraine in full.
How to use it
- Complete only the prompts that matter to your situation. Unknown or forgotten details can stay blank.
- Watch the live PDF preview update as you enter information.
- Download or print that exact PDF, or use the blank worksheet by hand.
- Review the result before sharing it. Remove anything you do not want another person to see.
A realistic way to use it
Select the communication difficulty that another person could observe, write one short instruction such as “Please speak slowly and give me time,” and add a contact plus the location of your phone’s Medical ID.
What the output means
The card communicates your stated preferences and details. It is not an emergency identification credential, advance directive, or substitute for professional emergency assessment.
Privacy by construction
The form works in browser memory. Calmraine does not receive the names, symptoms, treatments, cycle dates, workplace notes, contact details, or other text entered here. Analytics can record that someone started or exported the tool, but never the field values. Reloading or clearing the form removes the current entries.
Methodology and sources
The card is constrained to wallet size and short field lengths so critical text remains legible after printing and folding. The concept was informed by an aphasia card while the prompts and design are original.

If a headache or neurological symptom feels new, sudden, severe, or unsafe, use appropriate urgent medical care instead of waiting to finish a worksheet.
