Describe the work barrier before discussing a change.
A useful workplace conversation connects a specific situation to a specific task. This plan helps you describe that connection, select changes worth discussing, and agree on how the result could be reviewed.
How to use it
- Complete only the prompts that matter to your situation. Unknown or forgotten details can stay blank.

A realistic way to use it
A screen-heavy role might pair “long screen sessions” with the task that becomes difficult, request a discussion about screen breaks or lighting, and set a date to review whether the change helped.
What the output means
The output is a conversation plan, not a legal request, medical certification, or promise that a workplace will approve a particular adjustment.
Privacy by construction
The form works in browser memory. Calmraine does not receive the names, symptoms, treatments, cycle dates, workplace notes, contact details, or other text entered here. Analytics can record that someone started or exported the tool, but never the field values. Reloading or clearing the form removes the current entries.
