Start with a blank, spacious worksheet.

The printable version uses US Letter paper, selectable text, generous writing space, and the same prompts as the private builder.

Why this exists

Turn a long treatment story into a record someone can scan.

Names, dates, benefits, side effects, and reasons for stopping are easy to blur together. This worksheet keeps those details in a consistent order, using your own words and leaving uncertain details blank.

How to use it

  1. Complete only the prompts that matter to your situation. Unknown or forgotten details can stay blank.
  2. Watch the live PDF preview update as you enter information.
  3. Download or print that exact PDF, or use the blank worksheet by hand.
  4. Review the result before sharing it. Remove anything you do not want another person to see.

A realistic way to use it

For one previous treatment, enter the name you recognize, approximate dates, what improved, what side effects you noticed, and why it changed. Use another blank sheet for the next treatment.

What the output means

The PDF records one person’s recollection of a treatment experience. It does not compare medicines, judge effectiveness, or recommend starting, stopping, or changing treatment.

Privacy by construction

The form works in browser memory. Calmraine does not receive the names, symptoms, treatments, cycle dates, workplace notes, contact details, or other text entered here. Analytics can record that someone started or exported the tool, but never the field values. Reloading or clearing the form removes the current entries.

Methodology and sources

The fields separate treatment identity, timing, observed benefit, unwanted effects, and reason for change. That structure is informed by migraine appointment and treatment-history worksheets while using original wording and layout.
