Why this exists

Walk into the appointment with a short, usable agenda.

Migraine visits can compress months of changes into a few minutes. This checklist separates the decision you need help with, the facts that support it, and the questions you want answered so the clinician can scan the page quickly.

How to use it

  1. Complete only the prompts that matter to your situation. Unknown or forgotten details can stay blank.
  2. Watch the live PDF preview update as you enter information.
  3. Download or print that exact PDF, or use the blank worksheet by hand.
  4. Review the result before sharing it. Remove anything you do not want another person to see.

A realistic way to use it

A person whose attacks became more frequent can state that change as the visit goal, add a best monthly estimate, list current strategies, and finish with two questions about what to track and when to follow up.

What the output means

The PDF is an agenda and self-reported summary. It reflects only what you entered; it is not a medical record, diagnosis, or assessment of which treatment is appropriate.

Privacy by construction

The form works in browser memory. Calmraine does not receive the names, symptoms, treatments, cycle dates, workplace notes, contact details, or other text entered here. Analytics can record that someone started or exported the tool, but never the field values. Reloading or clearing the form removes the current entries.

Methodology and sources

The prompt order follows an appointment workflow: agenda first, supporting facts second, questions and follow-up last. It was informed by appointment-preparation resources from Migraine Canada and the American Migraine Foundation.

If a headache or neurological symptom feels new, sudden, severe, or unsafe, use appropriate urgent medical care instead of waiting to finish a worksheet.
