A diary does not assess urgency
Do not wait to complete a diary if you think you need urgent medical care. This template records what happened; it cannot diagnose a headache type, identify a cause, recommend treatment, or decide whether symptoms are safe.
Why eight weeks?
NICE guideline CG150 says that when a headache diary is used to aid diagnosis of primary headaches, the person should be asked to record a minimum of eight weeks. Recommended fields include frequency, duration, severity, associated symptoms, headache-relief medicines, possible precipitants, and relationship to menstruation. This is guidance for people over 12 in the UK, not a promise that eight weeks will produce a diagnosis or a universal rule for every appointment.
Keeping a diary should not become another overwhelming task. The American Migraine Foundation recommends focusing on information that will be valuable to you and your healthcare professional, and simplifying the record when needed.
What this diary cannot tell you
- It cannot diagnose migraine or another headache type.
- It cannot prove that a food, weather change, activity, menstrual timing, or other observation caused a headache.
- It cannot tell you whether to start, stop, repeat, or change a medicine.
- It cannot decide whether symptoms are urgent.
